Depends whether you mean pronouncing it "el oh el" or "loll"?

I might have said to someone: " 'el oh el' stands for ... ", but does that count for what you're asking about?

i mean actually using it in the same context as you would (or wouldn't) online.

It's always to make fun of people who actually would say it out loud.

I've said it out loud when I'm making a joke about online talking, or when I'm being snarky about someone who uses LOL too often.

it's an effective indicator, both online and off, when you want someone to know you're joking and your delivery doesn't indicate it.

It really is. I rely almost too heavily on emoticons too.

You're nearly guaranteed that the other person will laugh or at least smile if you say LOL out loud with a grin.

I don't even say it online. Online, I'm likely to say the same thing I say offline. Rather than "LOL" I'd say "heh" or something to indicate a chuckle.

Maybe I could do it like Spanish-speaking people do: jejejejeje

Is it better if I promise I have only used it in a heavily ironic fashion? ;)

I missed my intended radio button by a mile. I don't recall having said "Elle, oh Elle," out loud.
